    Insurance

Dr. Fischer is out of network with insurance.  She will provide a Statement of Insurance Reimbursement after the assessment for you to submit to your insurance company for reimbursement.  The Statement for Insurance Reimbursement will have all necessary billing codes (i.e., CPT codes) and diagnostic codes (i.e. ICD-10/DSM-5-TR codes) that the insurance company requires.

It is recommended that you contact your insurance company prior to the assessment to determine reimbursement rates if that if important to you.

    Payment

Dr. Fischer accepts cash and all major credit cards as forms of payment.  She also accepts payment directly on the client portal site via this page.  Payment is due at time of service.

    Cancellation Policy

If you are unable to keep your appointment for any reason, you must call at least 48 hours in advance; otherwise, you will be charged a late fee of $600 for the time Dr. Fischer has reserved for you.

If a feedback appointment (or any other 50 to 55-minute appointment) is missed, a $200 fee will be charged.

Please understand that it can be difficult to fill testing appointments last minute.  If you know you will not be able to attend an appointment, please contact me as soon as possible to reschedule.

    Good Faith Estimate

Under the law, health care providers need to give patients who do not have insurance, or who are not using insurance, an estimate of the bill for medications, items, or services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items ore services.  This includes related costs like medical test, prescription drugs, equipment, and hospital fees.

Dr. Fischer will provide you with a Good Faith Estimate in writing within one business day of scheduling your appointment.

If you receive a bill that is at least $400 or more than your Good Faith Estimate, you can dispute the fee.
